Patient Forms

Patients, we understand that medical paperwork can be cumbersome, arduous and a pain to fill out. In order to stay compliant with the standards required by your medical insurance providers we require patient paperwork be filled out for ALL new patient and follow up visits, even if the information remains unchanged from your previous visit. This allows us to properly document and allows Dr. Garoutte to formulate an up-to-date plan of care.
